Case 23 – the beginning

You are the medical team in the admissions ward.  You are referred a patient by a local GP with the following letter:

Dear medical registrar

Thanks you for accepting this 31 year old female who has had a swollen right calf for the last three days.  It is painful and the skin appears red.  Except for occasional thrush and gastritis she is normally fit and well.  She takes omeprazole 10mg per day and has no allergies.  I am concerned about a DVT and have performed a point of care test for d-dimer and it is positive.  She went to Ibiza six months ago but has not had any other foreign travel.
