Case 23 – update 3

Unfortunately following gynaecological review our patient is found to have cervical cancer.  She needs hysterectomy within the next week.  Following #teamhaem suggetstions she is given intravenous iron in an attempt to optomise her haemoglobin pre surgery and avoid the use of blood transfusion.  BD LMWH is used to avoid peaks in levels.  Unfractionated heparin could have been used with the advantage of a short half life and better reversibility if any bleeding concerns but due to lack of experience of ward staff and requirement of monitoring it is felt safer to avoid.

Questions:

  • How to you manage her anticoagulation pre-operatively?
  • How do you reverse LMWH?
