Case 61 – update 1

What have we found out so far?

We have a 15 year old girl who has presented with a few week history of easy bruising.

Blood tests have shows a microcytic anaemia with ferritin of 5 and a thrombocytopenia with a plt count of 11. Platelet function tests are normal.

Blood film shows occasional large plt. The plt appear normally granular. No fragments. Lymphocytes and neutrophils appear normal. Microcytic anaemia with occasional pencil cells.

The patient had a normal platelet count a few years ago.

What is your diagnosis? Are there any other tests you would do?would you like any other history? What would your management plan be?
